What is Air Abrasion?
Air abrasion is a minimally invasive technique that uses a fine, precisely controlled stream of pressurized air and tiny abrasive particles — typically aluminum oxide or a biocompatible alternative — to remove decay, prepare tooth surfaces, and treat early lesions without touching the tooth with a rotating bur or drill.
The particles are directed at the specific area being treated, removing material precisely and conservatively. Because there is no rotary instrument making contact with the tooth, there is no heat generated, no vibration transmitted through the jaw, and no sound beyond a gentle hiss. Most patients describe the sensation as a light stream of air — nothing more.
The removed particles and debris are simultaneously vacuumed away, keeping the area clean and dry throughout the procedure.
What We Use Air Abrasion For
Dental Sealants — More Effective Placement
Dental sealants are thin protective coatings applied to the grooves and pits of back molars to prevent cavities from forming in those hard-to-clean areas. The effectiveness of a sealant depends entirely on how well it bonds to the tooth surface — and that bond is significantly stronger when the surface is properly prepared before placement.
Air abrasion is ideal for sealant preparation. Rather than using acid etching alone — which can be unpredictable on some tooth surfaces — air abrasion micro-abrades the enamel surface, creating an optimal texture for the sealant to adhere to. The result is a stronger, longer-lasting bond that keeps the sealant in place and performing as intended for years longer.
This is particularly valuable for children getting their first permanent molars sealed — the procedure is completely painless, takes only a minute or two per tooth, and requires no injections whatsoever. Most kids don’t even realize anything has happened.
Small and Superficial Cavities — Kids and Adults
For early-stage cavities that haven’t progressed deeply into the dentin, air abrasion allows us to remove only the decayed portion of the tooth with extraordinary precision — preserving more healthy tooth structure than a conventional drill ever could.
Because the instrument never actually touches the tooth and generates no heat, many small cavity treatments with air abrasion require no local anesthesia — no needle, no numbing, no waiting for the injection to take effect. Whether we’re placing a CEREC same-day crown, a porcelain veneer, a Lumineer, or a ceramic inlay or onlay, the strength and longevity of the restoration depends heavily on how well it bonds to the underlying tooth.
Air abrasion micro-etches the tooth surface at a microscopic level — creating a texture that dramatically increases the surface area available for bonding and removes any contamination, old bonding agent, or surface debris that could compromise adhesion. Studies consistently show that air abrasion surface preparation produces bond strengths that are equal to or greater than traditional acid etching alone, and the combination of air abrasion followed by acid etching produces the strongest bonds of all.
For crowns and CEREC restorations: Air abrasion prepares the prepared tooth surface for optimal crown seating and bonding — particularly important for all-ceramic crowns where bond strength directly affects fracture resistance.
For porcelain veneers and Lumineers: The minimal tooth preparation involved in veneer placement makes surface quality critical. Air abrasion micro-etches the enamel precisely without removing unnecessary structure, creating an ideal bonding surface for the ceramic facing.
For inlays and onlays: Ceramic inlays and onlays rely on adhesive bonding rather than mechanical retention alone. Air abrasion preparation of both the cavity walls and the fitting surface of the restoration significantly improves bond strength and long-term clinical performance.
For bonding repairs: When repairing or replacing existing composite bonding, air abrasion removes old bonding material and prepares the surface for new composite without damaging surrounding enamel.
Air Abrasion in Pediatric Dentistry
Children and air abrasion are a natural fit — and we use it extensively in our pediatric patients for exactly this reason.
The absence of the drill sound, the absence of vibration, and in many cases the absence of any injection makes the experience of dental treatment dramatically less threatening for young patients.
